Amy Blanc is the director of social work, where she leads a team dedicated to supporting residents and their families through compassionate, person-centered care. She specializes in care planning, discharge planning, and advocacy for the residents, ensuring residents receive the resources and support they need to maintain dignity and quality of life. Amy is committed to interdisciplinary collaboration and creating a supportive environment that promotes both resident well-being and team excellence.
She is an experienced social worker for the past 14 yrs. She earned her degree in Psychology. She worked her way up from a social work staff to a director of social work. When she is not working, she is spending time with her husband and two sons at a sporting event or family vacations.

Samantha Smith, BSN, RN, is an experienced nursing leader with over 20 years of progressive healthcare experience. As Director of Nursing, she is dedicated to delivering high-quality, resident-centered care while promoting a supportive and collaborative work environment.
Her background in regulatory compliance, quality improvement, and clinical operations allows her to lead with both compassion and strategic insight. Samantha is passionate about empowering her team and advancing excellence in long-term care.
Brian Reese DPT
Director of Rehabilitation
Dr. Brian Reese, PT, DPT, is a seasoned healthcare leader and Doctor of Physical Therapy with over 22 years of experience in clinical practice and 12 years as a Director of Rehabilitation. A graduate of Utica University, Dr Reese is dedicated to helping patients safely transition from hospital to home through individualized, goal-driven care.
Throughout his career, Dr. Reese has specialized in delivering high-quality rehabilitative services while building and mentoring high-performing interdisciplinary teams. With extensive experience in post-acute recovery, he leads a therapy team focused on restoring strength, mobility and independence following illness, injury, or surgery. As a Director of Rehabilitation he has led program development, operational strategy, compliance initiatives and performance improvement efforts-consistently enhancing both patient outcomes and departmental efficiency.
Dr. Reese is committed to evidence-based practice, compassionate patient engagement, and creating rehabilitation programs that promote optimal outcomes and a successful return to daily life.
